What is the quality of the phone calls to outside landlines?

(Note: I am specifically asking regarding phonecalls originating from, and destined to, locations outside of North America) I am asking this as I have experienced problems with other internet based phone systems (VoIP), eg. Skype, et cetera. Problems experienced previously: (note: problems have been experienced on the listeners end of the phone call) Problems with the phone calls themselves: - "tinniness" of the voice. - extreme delays (always, to varying extremes) - interrupted phonecalls - premature disconnection - connection to the wrong landline! (the phone numbers have been correct). Problems with the services: - absolutely zero customer service provided for the solution of problems (Skype is an excellent example of this, but also other services). - monies paid in have not been credited to my account ballance (again Skype is an excellent example, but also other services). et cetera - I hope you get the idea of my enquiries.
